In a large pot of water, add a bit of kosher salt to the water and cook the pasta per package instructions. Drain pasta and reserve about 1 cup of pasta water for making the cream sauce. Do not rinse the pasta.
Over medium heat melt the butter in a Dutch oven or heavy skillet. Add the garlic and cook until fragrant, about 30 seconds. Stir with a wooden spoon to avoid burning.
Stir in the heavy cream, chicken broth, and lemon juice or lemon zest and stir to combine and cook over medium-low heat until the sauce is warm, but not boiling.
In a separate bowl, mix together the grated Parmesan cheese and flour. Toss until the cheese is coated with the flour.
Stir in the cheese mixture to the sauce and continue cooking over medium-low. Continue stirring to mix and melt the cheese. As the cheese melts the sauce will begin to thicken.
Add the cooked salmon and capers and stir until mixed.
Stir in the fettuccine, mix to coat, and serve warm.
